Fordyce Granules

Definition

Fordyce granules (also known as Fordyce spots) are ectopic sebaceous glands—oil-producing glands that appear in areas where sebaceous glands are not typically found.These are small, pale yellow or white bumps that can appear on the lips, inside the cheeks, and on the genitals.


Aetiology

This condition is developmental in nature and is considered a normal anatomical variation.


Clinical Features

  • Commonly observed on the lips and buccal mucosa.
  • Appear as yellowish-white lobules, typically grouped in clusters.
  • The surrounding tissue appears normal and symmetrical.
  • Lesions are benign and asymptomatic.
  • Typically emerge after puberty.

Differential Diagnosis

  • Oral candidiasis – may present as white or red patches but differs in texture, symptoms, and response to antifungals.

Management and Treatment

  • No treatment is necessary as the condition is harmless.
  • Reassure the patient that it is a common and benign variation.

If removal is desired for cosmetic reasons, the following options may be considered:

  • Micro-punch surgery – removes spots quickly and efficiently.
  • Laser treatment – e.g., CO₂ or pulsed dye lasers.
  • Topical therapies – though evidence for effectiveness is limited.
